This seems like a straight forward piece of equipment but I must be missing something.  The set I have are aluminum 12mm hexes that ride on their own bearings that fit the stock spindles/axles. I assume they pretty much all look the same.  
 

The problem is that when getting the wheel nuts snugged up, the bearings bind and the wheels don’t spin freely. leaving them loose enough to roll as they should makes a wheel departure very likely. 
 

What am I missing?

I don't think you are missing anything really. They are a compromise at best and that is pretty much how they work. Using thread lock on the wheel nut sometimes helps as then you don't need to do it all the way up to prevent it parting company with the car when you drive.

Try mounting them the other way around. With the small round part against the steering knuckles and the hex side against the wheel. As long as the nylon ring of the locknut is engaged loosing a wheel is unlikely.

I fitted those and had to open up the bore on the wheel centre to match the axle. I think it was on my hotrod or Wild One when I fitted HPI vintage rims. Span fine once I’d added a mm or two. 
 

@Tamiyastef is likely correct and the hexes will want flipped.

Yeah these don't really work great per @TurnipJF's response unfortunately. As soon as the nut is tightened it clamps the whole assembly together and prevents any rotation. 

In theory if the threaded section acted as a back-stop for the nut and this allowed enough space so the hex/wheel wasn't under tension, this would work - but there will still be friction between wheel face and the back of the flanged locknut, which is not ideal and will ultimately mean that the nut will come undone eventually (sometimes, very quickly!).

I have the reverse of this issue and would rather fit 2WD rims onto a hex-equipped TD2. Does anyone have any ideas how that might be achieved? I almost need a fat stub axle insert or entirely different uprights, but I'm not sure what would fit the TD2 platform.

Could try fitting DT02 c hubs and carriers. Or the 201 parts if you can get them. The 201 c hub does fit the TD2 arms.
